Guidelines for submitting a request:

  • To request support from ZP Better Together, please fill out the form below in its entirety.

  • Please attach your company/organization’s W-9 Form to this form.

  • A member of the ZP Better Together team will contact you upon receiving the form.

  • Completing this form does not guarantee sponsorship.

  • Your organization must have a Federal Tax ID (EIN) to request this.

  • All payments will be made via ACH transfer.

Any contribution awarded does not obligate your organization in any way to utilize any of ZP Better Together’s services or endpoints.
